How to Register Your Child with Creative Child Day Home Agency
Registering your child with Creative Child Day Home Agency is a great way to ensure they receive high-quality care in a warm, home-like environment. Please follow the steps below to begin the enrollment process:
1. Submit the Childcare Inquiry Form (on this page)
Please note: Submitting this form does not mean your child is registered—this is the first step in the process.
*If you’ve already chosen an educator, please include their name in the “Any Other Details You Would Like To Provide” section.
2. Account Access & Initial Review
When you submit your inquiry, your email will be used to create a username for our website.
Once your access is approved, you will receive:

An auto-generated password via email to log in to the site

A Welcome Email containing your next steps for enrollment
We will approve access once we have sufficient information to proceed—so please provide as much detail as possible on the inquiry form.
3. Connect with Educators
We encourage you to contact and interview potential childcare educators through our agency to find the best fit for your child and family.
4. Choose Your Childcare Educator
Once you’ve made your selection, inform us so we can continue with the registration process.
5. Complete the Enrollment Forms
Once your access is approved, log in to your Parent Dashboard and complete the following:

6. Submit Required Verification Documents
Please provide the following:

Your child’s Birth Certificate
-
Immunization Record
-
One piece of Government-Issued ID (parent or guardian)
You may submit documents in one of two ways:

Email scanned copies to: creative.dayhome@gmail.com, or
-
Visit our office in person with the originals for a quick 5-minute verification (we do not retain copies).
7. Sign the Contracts
Once documents are verified, we will send you two contracts via Adobe E-Sign:

Agency–Parent–Educator Contract

Educator’s Additional Terms Contract
Please review and sign both to finalize your child’s registration.
Please Note:
In order for us to process your enrollment and prepare for your child’s care, all required documents must be submitted at least 5 business days before your intended start date.
Late submissions may result in delays to your child’s start in care. We appreciate your cooperation in helping us prepare effectively for your child’s arrival.
Care cannot begin until all paperwork is complete and verified by the agency. If documents are delayed, your child’s start date may need to be adjusted.
8. Begin Care!
After the contracts are signed, your child will be officially ready to begin care with their selected educator.
